Leave of Absence Form (PDF)

Members who expect to be absent for two or more calendar months are reminded to file a "Leave of Absence" request with the Membership Coordinator. Most of your dues payments are waived during a Leave of Absence -- the club only collects that portion covering your membership in the local dancers' association (which should be paid when you submit your Leave of Absence request). See the form for more details. A Leave of Absence can be requested for a minimum of two and a maximum of six months.
